By checking the box below, the individual executing this agreement has read and agrees, on behalf of the individual or entity for which it is executed, to be bound by the Terms and Conditions of this registration contract, incorporated herein by reference, and further warrants that they have authority to execute this agreement by electronic signature and full authority to act for the individual or entity entering into this agreement. This document, when signed by the attendee constitutes a binding legal agreement. By signing this agreement, the Attendee agrees that it has received, read and agreed to the Terms and Conditions below.

DATA COLLECTION, STORAGE & SHOW INFORMATION
By registering for this event, you consent to collect and store your data on this form in the United States. You consent to receive all mail, email, or phone communications from Event organizers regarding Event information or updates. Event organizers do not sell or share attendee, exhibitor, sponsor or speaker information, unless provided permission below. If you were contacted about a list, consider the message as spam as it is fraudulent.

LIABILITY, HOLD HARMLESS
Registrant acknowledges and agrees that Event Organizers shall not be liable for any claims, losses, damages, death, injuries, communicable disease, illness, or other liability which may be sustained by any person who may be on the premises for the Event, including but not limited to any agent, employee, representative or guest of the registrant, or any other person or entity. The registrant agrees that it will hold harmless Event organizers from any and all claims, losses, damages, death, injuries, communicable disease, illness, or other liability whatsoever, whether to property, person or otherwise, that may arise in connection with attendance at the Event. In no event will Mayfield Renewables, Inc be liable to the registrant, whether in contract or tort, for any amount in excess of the fees paid by registrant. In no event shall Mayfield Renewables Inc be liable, whether in contract or tort, for any indirect, consequential, exemplary, punitive, or special damages or awards, including but not limited to lost profits.

QUALITY ASSURANCE POLICY
Mayfield Renewables is committed to delivering high-quality workshop sessions across all formats: in-person, online, and virtual. Ensuring that each session meets rigorous standards to provide an enriching learning experience for participants. We continuously monitor participant satisfaction and identify areas for improvement through integrated continuous evaluation and
feedback mechanisms in our sessions.

ADMISSION AND ENROLLMENT POLICY
Online classes are open to the public for anyone who registers. While descriptions are provided to assist in understanding the knowledge level, there are no prerequisites for classes, allowing individuals to self-select their level.
Mayfield Renewables Inc strives to be an inclusive institution and strictly prohibits discrimination of any kind based on race, religion, sex, national origin, ancestry, disability, marital, family, or pregnancy status, sexual orientation, gender identity, gender expression, citizenship status, or age.

CODE OF CONDUCT POLICY
Attendees of the Mayfield Renewables Education Summit and related activities agree to act in a respectful manner throughout the event. If an attendee is disrespectful or disruptive during class, they may not receive NABCEP credit for workshops, and may be asked to leave at the discretion of the instructor or Event organizers.
Guns, weapons of any kind, recreational drug use, any illegal activity, or excessive alcohol consumption are not allowed on the premises of any events organized by Mayfield Renewables, Inc.

COMPLAINTS AND GRIEVANCES POLICY
All inquiries can be directed to education@mayfield.energy. Any concerns will be addressed within 2 working days, along with a plan for resolution. If, during the event, an attendee has a complaint or grievance, please contact the Event organizers directly. Mayfield Renewables Inc is not responsible or accountable for the details of the space, food, or beverage provided by third parties in conjunction with the Event and much is out of the organizersโ€™ control.

REFUND AND CANCELLATION POLICY
Requests for refunds must be submitted via our education@mayfield.energy. Upon receipt, requests will be responded to within 2 working days. Refunds are not guaranteed, but the Event organizers will do their best to provide fair and reasonable refund and cancelation policies to accommodate reasonable requests.
Photography & Videography(Required)
PHOTOGRAPHY AND VIDEOGRAPHY
Event organizers plan to include professional photography and videography to capture the Event and participants. By registering for the Mayfield Renewables Education Summit and/or Welcome Reception, registrants grant permission to the rights of their image, in video or still, and of the likeness and voice as recorded on audio or video. Registrants waive the right to inspect or approve the finished product wherein their likeness appears. This material will be the copyright of Mayfield Renewables, Inc and may be used in diverse settings by the Event organizers, Mayfield Renewables, or the Agency of Record. Photo or video may be used for any of the following but not limited to; presentations, courses, marketing and promotion. Stills and video will not be sold or shared with any third-parties.
